Application of Death Benefit Claims (DBC Form 001); Certified True copy Death Certificate of the deceased barangay official; Certified True Copy of Marriage Contract (if claimant is spouse of the deceased barangay official); Certified True Copy of the Birth Certificate (if claimant is son/daughter); Certified True Copy of Birth Certificate (if barangay official is single and claimant is parent/brother/sister); Affidavit of Guardianship (if claimant is minor); Special Power of Attorney (auhtorizing one of the legal beneficiaries to claimant the benefit); Photocopy of government issued Identification Card (Voter's ID, Driver's License, Postal ID or certificate from barangay)
